The Hillside Centre offers intermediate care between being discharged from hospital to returning home. It is run by Hereford Primary Care Trust and so there is no cost - length of stay can be up to 6 weeks.

RESIDENTIAL HOME FOR MENTALLY IMPAIRED

OAK HOUSE - provides 24-hour care by a multidisciplinary mental health team made up of nurses, a psychiatrist and a psychologist.   

 

Oak house is an NHS run residential unit located in a street near the centre of Hereford providing rehabilitation and psychosocial interventions to 10 men and women with severe, long-term mental health problems. The team uses a variety of medical and other interventions including Behaviour Family Therapy and Cognitive Behaviour Therapy.  It also provides continuing support to Service Users after they leave Oak House. Relative Support Group for relatives of Oak House residents meets once a month at 27a St Owen Street, Hereford.

 

REFERRALS: Oak House is a secondary mental health service and as such only considers referrals of people who are already in touch with Mental Health Services. Those who are referred are screened initially for suitability followed by an assessment period of 3 months, which if successful leads to a placement offer.

Woolbridge Court is a large hostel for single homeless people with special needs, particularly mental health needs. It takes referrals of people aged 18-65 and provides furnished bedsits with large communal living area and laundry. Evening meals and breakfast are provided.

 

Elmhurst

Elmhurst is a residential home that provides short-term breaks for Carers through day care and residential care.

 

Aston House

ASTON HOUSE is a 16 place Residential Home on the edge of Hereford. It has mostly single and some double rooms with three communal living rooms. The house offers a range of in house activities and participation in its daily management is encouraged. Most residents attend a variety of daytime activities and work placements in Hereford.

RESPITE/RELIEF CARE - Local

 

Respite Care at Ryelands Centre

This service is available for five days a week through the Autumn Circle day service for older people. Respite places are available on a pre-booked basis to enable carers to have time for themselves, either for a whole day or just time to go to the hairdresser.  Please telephone for further information.

St Michael's Hospice

The Royal British Legion is a charity to help those from the ex forces community and their dependants in time of need. It functions through a countrywide network of branches manned by volunteer members. The legion offers comradeship and support offering help through grants, holidays, retraining, job seeking, residential home accommodation, in one of its own homes and small business loans for the unemployed. Assistance can also be given on making war pension claims and advice on state benefits.
