Herefordshire Primary Care NHS Trust, an Occupational Therapist provides assessment and practical help with adjustment to illness and disability. A community service is available including advice on aids for daily living, adaptations and house extensions and the appropriate local service.
There are Occupational Therapists based in localities within the Herefordshire Primary CareTrust, offering a service to people of all ages in their own home, workplace, clinic or community hospital. This includes assessment for people with a wide range of disabilities, including specialised advice on equipment; advice and support on lifestyle and management for people and carers with continuing problems; assessment for major and minor adaptations to accommodation and providing liaison with Social Services and access to District Council grants.
