The Charter is produced jointly by all the NHS organisations and the Council in Herefordshire. The Charter provides information about some of the key standards of service which can be expected from all these public bodies. It also gives people a chance to make comments about their experience of the service they received from any or all of these organisations. The Herefordshire Better Care Higher Standards Charter sets out standards of service delivery across Health, Housing and Social Care. The standards cover services for adults that have long-term care needs.  There are also standards for informal carers. The Charter contains important information on contacting services and a response sheet to allow people to comment on and compliment the services that they have received. Please let us know if you require a version of this document in another format or language. The Department of Health also produces a national Long-Term Care Charter, which describes the policy and its aims.

ACCOMMODATION WITH SUPPORT:  Working with our housing partners we offer supported accommodation as a 'stepping stone' for people who wish to secure their own accommodation choice in the future. We provide a flexible support service throughout the week including evenings and weekends.


 

COMMUNITY SUPPORT:  We visit people with their own homes and provide flexible support throughout the week including evenings and weekends to facilitate access to community opportunities and resources.

 

The service offers support with accessing:  Community resources - Educational opportunities - Housing - Leisure pursuits - Training for work opportunities and assistance with:  Benefits - Budgeting skills - Developing social opportunities - Maintaining and developing chosen support networks - Practical tasks associated with daily living - Tenancy sustainment.

 

We aim to provide a supportive and enabling service to help people sustain an independent life in the community.

The Shires provides a comfortable and warm environment for fourteen residents. The service is registered as a specialist nursing home for individuals suffering with severe and enduring mental health problems. Individual care and support is given to maintain independence and enhance skills of daily living.  Individual care programmes are used to assist each resident to function at their maximum potential and all people involved in the resident's care meet regularly to review the care which is given according to the needs of the resident. Above all we are a home where high quality health care is provided to prevent any further deterioration resulting from mental ill-health. City and rural drop-in services for people experiencing social, emotional or mental health needs. We aim to offer a friendly place to come and meet other people who may be experiencing similar difficulties as well as support and information around benefits, housing and other mental health issues.  All the centres offer a mix of structured and unstructured group activities. Hereford, Leominster and Ledbury provide inexpensive meals and the other centres by arrangement.  All drop-in services offer educational and recreational courses throughout the year. The City Service and Ross Drop-in offers very low-cost alternative therapies by appointment only. A variety of activities are available to include trips to interesting places, Information Technology (IT) facilities and a mix of arts and crafts sessions.

 

DAY SERVICES:   Herefordshire Mind provides day services to people experiencing social, emotional or mental health problems.  They aim to offer a friendly place to come and meet other people who may be experiencing similar difficulties. Support and information around benefits, housing and general mental health issues.  Inexpensive meals are provided in Hereford and Leominster and are arranged in the other centres. Tea and coffee is also served.

The focus of the SHYP project is on working with 16-25 year olds living in Herefordshire who need help with support or accessing accommodation. The projects we manage range from Supported Housing in both shared and self contained accommodation that is modern, fully furnished and excellent value; Outreach support: Our workers support homeless young people in setting up flat/amenities/budgeting/help with maintaining tenancy, accessing local services/applying for benefits etc; Nightstop emergency accommodation that exists to provide high quality, sensitive and supportive emergency accommodation to homeless young people between the ages of 16-25 years in the homes of ‘host families’.  Lone parent support to young parents (aged 16-25 years). The lone parent outreach worker assists young parents to maintain their accommodation and signposts them to other relevant services/agencies.

The aims of CRISIS is to help vulnerable and marginalised people get through the crisis of homelessness and fulfil their potential and transform their lives. They have various ways of reaching their aims. Some of these are: developing innovative services which help homeless people to rebuild their social lives and practical skills and join the world of work and reintegrate into society, they do this by enabling homeless people to overcome acute problems such as addictions and mental health problems.

Shelter can offer a wide range of support to homeless families living in temporary accommodation and families at risk of becoming homeless. They also help individuals.

A Regional Care Advisory Service - these consist of teams of Regional Care Advisers who:Provide information and advice to families - answer crises calls and liaise with other professional service providers - promote and develop a full range of local services - identify suitable respite and residential care facilities - liaise with local branches and self-help groups - give talks and organise seminars and training days - provide speakers for training sessions - provide workshops for service providers and users such as health, social services, nursing homes, and residential care staff teams.

ASBAH is the leading UK registered charity providing information and advice about Spina Bifida and Hydrocephalus to individuals, families and carers.  Our unique services are targeted towards:

·         support to parents before and around the birth of their baby, or diagnosis of the disability

·         support to the child and family on educational matters

·         specialised information and help on health matters

·         helping young people access services to progress towards control of their lives

·         responding to the needs of adults with Spina Bifida and/or Hydrocephalus

Our staff also includes Specialist Advisers in education, health and continence management.
